Nordic Nanovector - Fundamentale forhold 2019 (NANO)

Enig, tror det skyldes at resultatene de legger fram påEANM er enda mer forskning “underveis”. Men denne gangen syntes jeg abstractet var endel mer utkrystallisert med en nådd milepæl/viktige delavklaring.

Hva tro dere nytten av resutlatet som nevnes i abstractet er/kan bli?

Kanskje du lager en tråd på HO som diskuterer det? Det er jo noen kapasiteter der som ikke er her.

Nja, hvis jeg husker rett så kjøpte verastem rettighetene til duvelisib i for en billig penge sent ute i utviklingsløpet. Tror de kjøpte rettighetene i 16. Jeg tror de i mindre grad har vært involvert i utviklingen av duvelisib.

present data from SWOG S0801, a phase 2 trial of R-CHOP (rituximab plus cyclophosphamide, doxorubicin, vincristine, and prednisone or prednisolone) chemotherapy followed by radioimmunotherapy consolidation with the 131iodine (I)-labelled anti-CD20 monoclonal antibody tositumomab and up to 4 years of maintenance rituximab in patients with advanced-stage, previously untreated follicular lymphoma. The main message was that the outcome, as measured by progression-free survival and overall survival after a median follow-up of 6·4 years (IQR 6·0–6·9), was better than in comparable studies in a similar patient population. Progression-free survival was 90% (95% CI 82–95) at 3 years, which compares favourably with the PRIMA study

(progression-free survival 75% [95% CI 71–79] at 3 years in the 505 patients who were randomly assigned to 2 years of rituximab maintenance after induction with R-CHOP, R-CVP [rituximab plus cyclophosphamide, vincristine, and prednisone], or R-FM [rituximab plus fludarabine and mitoxantrone]). If indeed this current study did have a better outcome than the PRIMA study, then this raises several important questions.

Stor entusiasme for nye behandlinger som utelukker cellegift. Positive resultater fra Archer-1 kommer garantert til å får stor oppmerksomhet.

The introduction of targeted agents for the treatment of non-Hodgkin lymphoma has resulted in substantial improvements in patient outcomes. In follicular lymphoma and mantle cell lymphoma, prolongation of overall survival has been demonstrated over the last several decades, attributed in part to the introduction of the anti-CD20 monoclonal antibody rituximab to standard chemotherapy.

Despite the improvement in survival with chemoimmunotherapy, cure has been elusive and the use of cytotoxic therapy is associated with short-term and long-term side-effects. Further development of non-cytotoxic therapeutics has sparked enthusiasm for chemotherapy-free combinations.

“For patients with follicular lymphoma enrolled in our trial, sequential R-CHOP, radioimmunotherapy, and maintenance rituximab resulted in excellent treatment responses and durability of these responses,” the study authors wrote in their published findings.

Det er pluss og minus: NANO sitt produkt passer perfekt inn i bildet, men det er også et veldig kompetitivt felt.

Helt enig, sterk konkurranse, men Nano ligger langt fremme i løypen med et bra produkt som er perfekt utformet og differensiert i forhold til konkurrenter.

Av de to NANO postere på EANM mener jeg den mest interessante poster av de to er: “Tumor Absorbed Dose and Changes in FDG PET Parameters in Non-Hodgkin Lymphoma Patients Treated With 177Lu-Lilotomab Satetraxetan”

“A total of 40 tumours from 15 patients (1-5 tumours per patient) were included”, dvs. de ser altså på "biopsier fra totalt 15 pasienter og noen har de tatt kun en biopsi fra, andre pasienter opptil 5 biopsier og biopsiene er tatt NB! ca. 3 måneder etter behandlingen og alt er målt ut fra standard prosedyrer og retningslinjer (blant annet EANM guidelines for tumour imaging v2).

Når det gjelder Betalutin så er jo det en behandling og så sammenligner man altså med “baseline” for pasientene 3 måneder seinere i denne posteren. Det de ikke sier eksplisitt er hvilke pasienter de har tatt en biopsi fra eller hvem de har tatt 5 biopsier fra. Har de tatt en biopsi fra de som har respondert fullstendig eller er det disse som de har tatt 5 biopsier fra? Mer om dette kommer nok, for dette vet de, men samtidig så gjør dette tolkningen av resultatene her ut fra denne posteren litt vanskelig.

Og det er egentlig veldig gode resultater de legger fram ved analysen av disse tumorene.

Min kommentar: “FDG PET-parameters” er en samlebetegnelse for de ulike parametre som brukes ved disse målingene:
-Standardized uptake value (SUV) max

  • SUVpeak
  • metabolic tumour volume (MTV)
  • total lesion glycolysis (TLG)

De ser etter 3 måneder en full reduksjon i SUVmax, SUVpeak, MTV and TLG for henholdsvis 18, 25, 26 and 26 tumours (dvs.en gjennomsnittlig forandring i FDG PET-parameters for SUVmax, SUVpeak, MTV and TLG were 56%, 64%, 78% and 75 %), men de finner ingen lineær korrelasjon mellom tumor absorbert dose og disse forandringer i målte parametre (No significant linear correlation was found between tumour absorbed dose and the relative change in PET-parameters)
Og med dette er noen kanskje blitt litt skuffet!? fordi de ikke ser noen særlig forskjell på de ulike doseringene og disse parametre?
Nei; det er ikke det de helt sier for:

Når de tar bort alle de med “complete resopons” (excluding patients that had a complete reduction in all included tumours) og så ser nærmere på tallene for hver pasient så ser de: " higher tumour absorbed doses resulted in more reductions in SUVmax for 6 of 8 patients".
Når det gjelder dosiemetristudiene vet vi at f. eks. 20/100 gir høyere absorbert dose (enn eks. 15/40), så vil jeg forsiktig konkludere:
Betalutin virker godt og målt ut fra ovennevnte parametre så får man uavhengig av styrke og tumor opptak ( 10, 15 or 20 MBq/kg of 177Lu-lilotomab satetraxetan) en god effekt i henhold til ovennevnte parametre, men man ser bedre effekt jo høyere tumor opptak på de pasienter som ikke oppnår “complete respons”.

Dette er altså målinger tatt etter kun tre måneder og som jeg skriver over så forteller de ikke hvem og hvor mange biopsier de har tatt fra de med ulike typer respons slik jeg ser det, men der de ikke oppnår “komplete respons” så er effekten bedre jo høyere den absorberte dose er. Studien sier heller ikke noe om f. eks. “duration of repons” knyttet til utprøvningene da målingene er tatt etter 3 måneder.
(Posterens navn er jo også: Tumor Absorbed Dose and Changes in FDG PET Parameters in Non-Hodgkin Lymphoma Patients Treated With 177Lu-Lilotomab Satetraxetan ) og kanskje de burde lagt til i overskriften på posteren: målt etter 3 months.

Ønsker å minne alle på at neste møte hos EMA angående kandidater for PRIME finner sted torsdag i neste uke. Frist for søknad var 29. august, altså etter abstract submission i forbindelse med ASH. Reglene rundt om de kan søke PRIME når de endelig data ikke er sluppet på ASH enda er litt uklar for meg. Vet ikke om dette ødelegger en eventuell posterfremleggelse ved ASH, men samtidig bør det jo være noen regler angående fortrolighet/taushetsplikt inne i bildet.

Spennende, men med tanke på hva LR sa på kvartalspresentasjonen har jeg ikke forhåpninger om det enda. De manglet relevant informasjon om noen av de tidlige pasientene, hvorvidt de hadde fått rituximab eller ikke. Tenker de hadde søkt allerede om det ikke var noe de ville avvente, men bare spekulasjon. Hadde vært meget hyggelig med en Prime overraskelse.

Husker du om Lisa sa dette i forbindelse med q1 eller q2? Om jeg ikke husker helt feil sa hun vel dette på q1(?) og at de håpte de hadde dette klart til ash. Ettersom submission til ash var 1.august og submission til PRIME var 29.august kan det være nærliggende å tro at dette er i boks. Men enig i at vi ikke skal forvente noen PRIME melding før til neste år :slight_smile:
